Before Your First Appointment

We need you to complete the required paperwork prior to your appointment.

Registration Form

This includes demographic information, such as your telephone, email, and insurance.

Submission: Once completed, you can fax the forms to 501.537.7412 or bring them with you to your appointment.
Portal Alternative: You may also fill out the registration forms on the Patient Portal where it says "New patients start here."

Patient History Form

This allows us to understand the problems you are having, past treatments, and medical problems.

Note: The history form is not available to fill out on the portal and must be completed manually.
